Personalized support to help you recover safely and confidently at home after a hospital or rehabilitation stay.
After hospital care, also called post-discharge care, is in-home support designed to help individuals recover safely after leaving a hospital, rehabilitation center, or skilled nursing facility.
Our caregivers provide hands-on assistance during the recovery period, helping manage daily tasks, follow discharge instructions, and reduce the risk of complications or readmission. We work alongside your medical team, so nothing falls through the cracks.
Going home after a hospital stay should feel like relief. But for many people, it’s actually when things get hardest. Without the right support in place, small gaps — a missed medication, an unsteady moment, a skipped follow-up — can quickly turn into something more serious.
According to NCBI, 1 in 5 patients experience an adverse event after discharge, and most of those events are preventable. Having someone there during those first days and weeks makes all the difference.
Our post-discharge care services are personalized to one’s needs, recovery plan, and daily routines.
Post-discharge care is often recommended when someone needs more support than their loved ones can provide during recovery, even temporarily.
This service may be a good fit if you or your loved one:
Fedelta’s in-home care services are designed to fit your routine, not the other way around. Whether recovery takes a few weeks or evolves into longer-term support, we build a plan that works for you.
As care needs change, we’re here to adjust alongside you, making it easy to continue care safely and comfortably at home.
With a local presence across Washington and Oregon, Fedelta proudly provides companionship care services to families in Seattle, Bellevue, Tacoma, Lynnwood, Olympia, Bellingham, Vancouver, and the surrounding communities.
After hospital care provided by Fedelta is non-medical in-home support: help with daily tasks, personal care, and recovery routines. Home health care involves licensed clinical services such as wound care, physical therapy, or skilled nursing, typically ordered by a physician. These services can complement one another during recovery.
In most cases, we can arrange care to begin the same day or within 24 hours of discharge. We understand that recovery doesn’t wait, and our team works quickly to get the right caregiver in place before you or your loved one arrives home.
The duration varies depending on the individual’s condition and recovery progress. Some clients need support for a few weeks following surgery; others transition into longer-term care as needs evolve. We reassess regularly and adjust the care plan accordingly.
In most cases, non-medical in-home care following a hospital discharge is not covered by Medicare or standard health insurance in Washington or Oregon. Medicare may cover short-term skilled nursing or therapy services ordered by a physician, but day-to-day support from a home care aide is typically a private-pay service.
In Washington, some individuals may qualify for assistance through Apple Health (Medicaid), certain long-term care programs, or the WA Cares Fund (beginning benefits in 2026 for eligible residents).
In Oregon, Medicaid and state-supported programs may provide assistance for eligible individuals who meet financial and care requirements.
We recommend reviewing your discharge paperwork, contacting your insurance provider, or speaking with a local care advisor to understand your options.
